VISITOR’S TRAVEL HISTORY AND HEALTH SCREENING

Due to the precautionary measures Headstart Language has taken to protect our community from the Coronavirus, all visitors must complete the Travel Declaration form below. You may be asked to pre-fill the form as it helps facilitate pre-qualifying entries into Headstart Language based on travel declarations. However, please note you will be required to complete the form again when you arrive at our premises due to the health screening questions that are mandatory to keep our community safe.
